Episode #4; 『Purgatory』

The chocolate is given to Sayaka by her mother shortly before performing the "Sachiko Ever After" charm with her friend, Naho Saenoki. Sayaka keeps it on her throughout her journey through Heavenly Host Elementary School, but refuses to eat it even when she's starving due to it being a memento. However, when her fate is ultimately left in the hands of Yoshikazu Yanagihori and his sledgehammer, her chocolate is eaten right in front of her by Sachiko Shinozaki.
